Creating Healthy Communities

Our Healthy Communities Program is designed to address the root causes of health disparities and promote equitable access to health resources. By leveraging the CHSC grant, we can implement evidence-based strategies that create lasting change in our community.

What is CHSC?

The Creating Healthy Schools and Communities (CHSC) grant, funded by the New York State Department of Health, aims to improve nutrition and physical activity environments across the state. It supports policy, systems, and environmental changes that promote healthier lifestyles by integrating Food Service Guidelines, enhancing active transportation, and increasing access to nutritious foods. Through partnerships with schools, municipalities, and community organizations, CHSC helps create sustainable, long-term health improvements in underserved communities.

Expand Access to Nutritious Food

Increase the availability of fresh, healthy, and affordable food in underserved communities through partnerships, infrastructure improvements, and educational initiatives.

Enhance Active Transportation and Public Spaces

Support the development of safe, accessible streetscapes, bike and pedestrian infrastructure, and public transportation enhancements to encourage physical activity and community engagement.

Strengthen Community-Led Health Initiatives

Empower local organizations, schools, and municipalities with resources, policies, and technical assistance to create lasting improvements in nutrition, physical activity, and environmental sustainability.

Our Projects

This year alone, Capital Roots’ Healthy Communities program has completed dozens of projects, adding to the hundreds we’ve accomplished since the program began. From expanding bike and pedestrian infrastructure to improving access to fresh, nutritious food, each initiative fosters healthier lifestyles and stronger communities. These projects have made lasting impacts, ensuring safer streets, greener public spaces, and better food access for residents across the Capital Region.
